Scheduling Appointments The clinic is consultative and designed to work in concert with primary care physicians. The initial evaluation is extensive and done with family members and other important caregivers present. The clinical portion of the evaluation involves history taking, cognitive testing, and physical examination. The family/caregiver portion of the evaluation involves history taking, education, problem-solving, and direction to necessary resources. In order to schedule an appointment, we will need a referral from the patient's primary care physician or neurologist. If the physician feels such referral is appropriate, he or she should contact our office and send the necessary records. Once this referral is received, our staff will contact the caregiver to schedule an appointment and send you a packet of information that will need to be completed and sent in prior to the appointment. MemoryCare is a consultative practice and requires a referring primary care physician for all patients enrolled: MemoryCare does not offer after hours or weekend coverage for urgent or emergency medical concerns and when such is deemed necessary call either the primary care physician after hours contact or the 911/emergency medical care system for needed assistance. Fees The clinical portion is covered by Medicare/Medicaid and other forms of insurance and the program accepts assignment. The patient will be responsible for any deductible amount that has not been met by their insurance or any co-payment that results from the clinical portion of the visit. An additional fee, not covered by Medicare or other insurance, is charged to the caregiver. This $460 per year caregiver fee is payable from the caregiver on the initial visit (the $75.00 deposit will be applied to the caregiver fee and the remaining $385 will be due at the time of the appointment unless other arrangments have been made). Please note that the caregiver fee cannot be paid by the patient as medically necessary services to the patient are covered by Medicare. This fee covers services for the supportive care and training of caregivers over a one year period. Such services include caregiver support, the use of our library resources, workshops, and our caregiver training and education course. This fee is necessary for our program to provide the quality of service appropriate for caregivers of memory impaired individuals; however, we pursue charitable funsing so that no family will be denied this service due to inability to pay. Our program is accessible by phone for assitance with caregiver problems between visits. The initial visit lasts three hours. Return visits average one hour.
